Please submit this form to register your student(s) for childcare at RPCA: 

• Before care (7:30 - 8:10am)

• Aftercare (3:15 - 4:30pm) 

• Friday care (7:30 - 4:30)

*Without this registration form completed, it is possible a teacher will not be present at the Academy.
